CASP4 is an inflammatory caspase that functions as the effector of the non-canonical inflammasome by mediating lipopolysaccharide (LPS)-induced pyroptosis. It also indirectly activates the NLRP3 and NLRP6 inflammasomes and acts as a thiol protease, cleaving a tetrapeptide after an Asp residue at position P1 to catalyze the cleavage of CGAS, GSDMD, and IL18. Within the non-canonical inflammasome, CASP4 is activated by direct binding to the lipid A moiety of LPS without requiring an upstream sensor, leading to the cleavage of GSDMD and IL18, followed by IL18 secretion through the GSDMD pore, pyroptosis of infected cells, and their extrusion into the gut lumen. Additionally, it indirectly promotes the secretion of mature cytokines (IL1A and HMGB1) downstream of GSDMD-mediated pyroptosis via activation of the NLRP3 and NLRP6 inflammasomes. CASP4 plays a crucial role in restricting S.typhimurium replication in colonic epithelial cells and is involved in cell death induced by endoplasmic reticulum stress and cytotoxic APP peptides found in Alzheimer's patient brains. Notably, GSDMD cleavage is not strictly dependent on the consensus cleavage site but relies on an exosite interface on CASP4 that recognizes and binds the Gasdermin-D C-terminal (GSDMD-CT) portion. By similarity, it also acts as an activator of adaptive immunity in dendritic cells in response to oxidized phospholipid oxPAPC, and excessive activation of the non-canonical inflammasome in brain endothelial cells may lead to blood-brain barrier breakdown.
